The fees for Provisional Status and Full Approval applicants for Bariatric Surgery Center of Excellence^® designation must be paid prior to the submission of an application. Provisional Status and Full Approval application fees are not invoiced unless requested by the applicant's accounting department. All other fees are invoiced for payment.

Fees may be paid by Wire Transfer from applicant’s bank or credit card (MasterCard or Visa only). All payments should include your SRC ID number, name of facility/surgeon and, when applicable, an invoice number.

Credit card payments require the completion of a Credit Card Payment Form. When needed, a Credit Card Payment Form can be requested by contacting . Credit card payments cannot be accepted over the phone. All credit card payment forms must have an authorized signature to be processed.

If paying fees by Wire Transfer, please contact to obtain a Bank Wire Notification form with Surgical Review Corporation’s bank information. A completed Bank Wire Notification form must be returned via fax to the accounting department at Surgical Review Corporation no later than the day funds are transferred. The fax number is listed on the Bank Wire Notification form.
