The fees for Provisional Status and Full Approval applicants for Bariatric Surgery Center of Excellence^® designation must be paid prior to the submission of an application. Provisional Status and Full Approval application fees are not invoiced unless requested by the applicant's accounting department. All other fees are invoiced for payment.

Fees may be paid by check or credit card (MasterCard or Visa only). All payments should include your SRC ID number, name of facility/surgeon, and applicable invoice number. Credit card payments require the completion of a Credit Card Payment Form. A Credit Card Payment Form and a W-9 Form can be requested by contacting . Credit card payments cannot be accepted over the phone. All credit card payment forms must have an authorized signature to be processed. Fees paid by check should be made payable to Surgical Review Corporation and mailed to:

PROVISIONAL STATUS FEES
BOLD™ ACTIVATION FEE			$875
	Billed once to the Practice. An invoice and BOLD participation documentation are sent following notification of Provisional Status. Payment and all required documentation must be returned within 90 days for BOLD activation.
BOLD ANNUAL DATA SUBMISSION			$875
	Billed annually, the invoice is sent to the Provisional Status Hospital/Freestanding facilities until Full Approval BSCOE designation is received. It is up to the Center (Hospital/Surgeon/Practice) to determine how this cost is shared.
BSCOE PROVISIONAL STATUS APPLICATION AND EXTENSION FEES
	Provisional Status is effective for up to two years. Extension of Provisional Status requires payment of an annual extension fee.
		Provisional Status Application - Hospital/Freestanding facility	$6,250
		Provisional Status Extension - Hospital/Freestanding facility (provides one-year extension)	$6,250
		Provisional Status Application - Surgeon	$500
		Provisional Status Extension - Surgeon (provides one-year extension)	$500

FULL BSCOE ANNUAL PARTICIPATION FEES
	BOLD Data submission fees are included.
BSCOE FULL APPROVAL APPLICATION/RE-APPLICATION
	Hospital/Freestanding facility
		First-year Annual BSCOE Participation Fee. As a new applicant, this is the first year of a three-year cycle. Initial site inspection fees are included. NOTE: Applicant surgeon(s) must also be compliant with BOLD prior to submission of Full Application. (Fee must be received before application can be submitted)	$11,250
		Second-year Annual BSCOE Participation Fee. (Fee will be invoiced.)	$2,250
		Third-year Annual BSCOE Participation Fee. (Fee will be invoiced.)	$2,250
	Surgeon
		First year Annual BSCOE Participation Fee. As a new application, this is the first year of a three-year cycle. NOTE: Applicant surgeon(s) must also be compliant with BOLD prior to submission of Full Application. (Fee must be received before application can be submitted.)	$1,000
		Second-year Annual BSCOE Participation Fee. (Fee will be invoiced to the surgeon.)	$500
		Third-year Annual BSCOE Participation Fee. (Fee will be invoiced to the surgeon.)	$500
RENEWAL ANNUAL BSCOE PARTICIPATION FEES
	This is a three-year renewal cycle. Additional site inspection fees apply. BOLD Data Submission Fees are included. All renewal fees are invoiced prior to submission of renewal.
	Hospital/Freestanding Facility
		First-year of three-year renewal cycle - renewal site inspection fee applies	$6,750
		Second year of three-year renewal cycle	$2,250
		Third year of three-year renewal cycle	$2,250
	Surgeon
		Each year of three-year renewal cycle	$500

SITE INSPECTION FEES
Renewal Site Inspection			$850
	Applies to the renewal-year site inspection. Fee applies per site inspector and assumes a one-day inspection. Any additional inspection days required will be billed at $425 per day per site inspector.
Additional Surgeon Site Inspection			$1,500
	Applies when Full Approval surgeon applicant is added to an existing BSCOE Center
Return Site Inspection			$2,500
.	Applies when BSRC requests an additional site inspection
Re-inspection After Cancellation			$3,500
	Applies when the site inspector must return for inspection after an inspection is cancelled due to lack of preparation by the applicant.
